I certify that my child is in good health and physical condition to fully participate in all Rugcutterz Danz Artz Inc. programs and activities, including dance, fitness, and summer camps. All current medical conditions, allergies, and required medications have been communicated and will be updated as necessary. I understand that participation involves inherent risks, including physical injury and exposure to communicable diseases (e.g., COVID-19).

I release Rugcutterz Danz Artz Inc., its instructors, directors, managers, and owners from liability for any injury, illness, or damages. I acknowledge that Rugcutterz Danz Artz Inc. is not responsible for disruptions due to events beyond its control, including natural disasters, pandemics, or other force majeure events. Programs may be rescheduled or adjusted as feasible.

I grant permission for Rugcutterz Danz Artz Inc. to photograph and/or record my child for educational, promotional, and advertising purposes unless I provide written notice opting out.

I understand that disruptive or unsafe behaviour may result in my child’s removal from a program. I agree with the program cancellation policies. By registering for Rugcutterz Danz Artz Inc. programs, I agree to these terms and conditions.
